Definition

Decision Traceability is the ability to reconstruct how and why a particular decision was made by documenting the evidence, assumptions, alternatives, participants, evaluation criteria, and reasoning that supported the final outcome. Rather than relying on memory or informal discussion, Decision Traceability creates a transparent record of the decision-making process.
